Health & HIV Care
Community-based HIV prevention, care and adherence support, including referrals, peer groups and home visits for people living with HIV.
Action for Community Care (ACC) works with government, communities and partners across Tanzania to strengthen health, education, livelihoods, social protection and environmental sustainability so that children, youth, women, elderly people and people living with HIV can live with dignity and hope.
ACC combines gender-responsive and community-based approaches to address poverty, inequality and health challenges, ensuring no one is left behind.
Our programmes are designed with communities and delivered through local structures. We support households to access health, education and economic opportunities while strengthening protection systems and promoting environmental sustainability.
Through close collaboration with District Councils, ward and village leaders and health facilities, our work complements national strategies and strengthens existing government systems.
Building the capacity of households, caregivers and community volunteers to make informed decisions, demand services and support one another.
Strengthening referral and linkage systems so that vulnerable groups can access health, education, social protection and livelihood services without barriers.
Working with leaders, influencers and networks to challenge harmful norms, promote gender equality and advocate for inclusive, sustainable policies.
ACC’s programmes reach families in their homes, schools, health facilities and communities – helping them access care, protection and opportunities.
ACC’s mission and vision guide how we plan, partner and measure our work – always with the most vulnerable at the centre.
To support and empower vulnerable and disadvantaged communities in accessing health, education, economic growth, social protection and environmental services, using community structures and strong partnerships.
ACC envisions a Tanzania where vulnerable children, youth, women, elderly people and people living with HIV enjoy improved health, opportunities and a safe environment that enables them to thrive.
Our complementary programmes address the multi-dimensional challenges facing vulnerable groups – providing them with the tools and support they need.

Volunteers, community champions and partners are at the heart of ACC’s work. Your time, skills and support help us reach more families with care and hope.
Behind every statistic are real people and families whose lives have changed. Here is one of many stories shared through ACC’s programmes.
Since ACC started visiting our village, my children have been able to stay in school and receive regular health check-ups. The community volunteers visit us at home, encourage us to join savings groups and help us get support at the health facility. We feel seen, supported and no longer alone.
